UPLIFT HEALTH FAQ

Uplift Health is not currently taking on patients for primary care.  However, we can provide you with the support and knowledge of a primary care physician as necessary to complement the care you are currently receiving from your PCP and or specialist.

Uplift Health offers a wide range of standard and specialized tests to help with diagnosing and tracking results which are not included in the membership. Some of the tests commonly ordered include comprehensive stool testing, comprehensive cardiometabolic panels, autoimmune antibody testing, food sensitivity and allergy panels, acute and chronic infection testing (tick-borne, viral), hormone testing, sleep study, genetic and epigenetic testing, mold and chemical testing, heavy metal testing, cancer screening tests, and micronutrient testing.

Standard test orders can be covered through your health insurance or via direct pricing through our discounted lab services with Quest.  We have standard lab testing available through Quest labs, Trinity Health, Spectrum Health, and U of M West.  (use logo of each health system on website)

Some specialized lab testing may be partially covered by insurance but will depend on the test ordered, current diagnosis, and patient’s insurance coverage.

Uplift Health has lab ordering and electronic medical record privileges with Trinity Health, U of M West, and Spectrum Health.  Patients will need to sign a medical release of records in order to provide this level of communication.

Common treatments utilized at Uplift Health include: dietary optimization, nutriceuticals and supplements, movement/exercise, sleep optimization, guided meditation, stress management, fasting/fasting-mimicking diet, acupuncture, and prescription medications if needed.

• Uplift Health does not participate in any health insurance plan and has opted out of Medicare/Medicaid plans.
• If you have an FSA (flexible savings account) or HSA (health savings account), it may be used to pay for acupuncture and or our membership fees (contact your account representative to confirm). Uplift Health will not submit insurance claims for you, but may assist you with the documentation needed to help you submit on your own, if requested. 

• Uplift Health is not health insurance and does not replace your health insurance plan. We recommend keeping your health insurance coverage for emergencies and other covered care.

$150 for initial appointment

$95 per follow-up appointment

$450 per 5 acupuncture package

• Adult New Patient Bundle: $700 (90 mins initial in-person visit, three 30 minute follow-ups virtual or in-person)
• Pediatric New Patient Bundle: $600 (up to 180 mins total, spaced over 3-4 visits, time and number of visit may vary)
• Additional Follow up visit (1 hour): $250
• Additional Follow up visit (30 minutes): $125

It is a 1 year membership program with Uplift Health designed to help patients with chronic conditions including obesity, heart disease, diabetes, hypertension, autoimmune disorders, gastrointestinal disorders who are seeking ways to utilize functional medicine, nutritional, integrative medicine approaches to change or reverse their disease process.
